CrimeaPRESS reports:
The Department of Labor and Social Protection of the Population of the Simferopol Administration provides targeted financial assistance for dental prosthetics in accordance with the Decree of the Council of Ministers of the Republic of Crimea dated March 01, 2016 No. 80 “On Approval of the Procedure for Providing Targeted Financial Assistance to Citizens in Difficult Life Situations”.
Citizens who receive a federal social supplement to their pensions in accordance with Art. 12.1 of the Federal Law of July 17, 1999 No. 178-FZ “On State Social Assistance”.
Financial assistance is provided no more than once every three years in the amount of actually incurred costs, but not more than 15,000 rubles.
List of documents for receiving one-time financial assistance:
- application for financial assistance;
- a copy of the passport of a citizen of the Russian Federation or other document proving the identity and place of residence, with the provision of the original for review;
- original certificates of income of each family member for the last six months before applying, which contain a complete list of income (wages, bonuses, scholarships, compensation payments of all kinds, income from renting out housing, unemployment benefits, dividends, interest and other payments) . In case of no income, the applicant provides copies of work books (1 page and pages with the last two entries);
- a copy of the document confirming belonging to a preferential category (if any), with the provision of the original for review;
- copies of documents issued by a doctor of appropriate qualification of the state healthcare institution of the Republic of Crimea, on the need for dental prosthetics with an expense estimate attached; confirming the performance of dental prosthetics (payment documents on the fact of payment for the performed dental prosthetics);
- current account opened with the bank to receive the payment (if any).
