Crimean doctors performed a unique one-stage operation on the vessels of the heart and neck

In the Department of Cardiac Surgery of the Multidisciplinary Republican Medical Center of the State Budgetary Institution of Healthcare of the Republic of Crimea «Republican Clinical Hospital named after. N. A. Semashko, Crimean doctors performed a unique operation for the region on the vessels of the neck and heart. According to the press service of the Ministry of Health of the Republic of Crimea, this is a simultaneous operation for simultaneous correction of heart pathology (coronary bypass grafting) and pathology of brachiocephalic vessels (carotid endarterectomy).
The patient was admitted with ischemic heart disease with a critical lesion of the coronary arteries, with a low ejection fraction. The ejection fraction, that is, the percentage of the volume of blood ejected into the vessels from the ventricle of the heart during each contraction, the patient had — 30%. Two arteries of the heart are completely impassable — occluded. And one vessel had a critical 80% lesion. The patient had the fourth functional class, which is characterized by the complete inability of the patient to any kind of load. In addition, during the additional examination, it was found that the patient had a completely obstructed carotid artery on one side and, on the other side, it was suboccluded with an obstruction of more than 80%., — said the chief freelance cardiovascular surgeon of the Ministry of Health of the Republic of Crimea, candidate of medical sciences, head of the department of cardiac surgery of the Republican Clinical Hospital named after. N. A. Semashko Ivan Davydov.
Previously, Crimean doctors either sent such patients to federal clinics or operated sequentially: first they performed surgery on the neck, then surgery on the heart. However, in this case, the transfer was not possible due to the severity of the patient’s condition, and separately, the patient could not endure these operations due to the critical damage to both the neck vessels and the vessels of the heart.
This patient could not be operated on in the first stage on the neck because there was a high risk of developing a myocardial infarction and could not be operated on for the heart in the first place because he would have a high probability of having a stroke. After consultations with the main external specialists, it was decided to perform a one-stage operation on the basis of our clinic. This is an all-Russian tendency to perform simultaneous intervention on the carotid arteries and heart surgery, but in the Crimea such an operation was performed for the first time.— said Ivan Davydov.
The operation lasted 4.5 hours and passed without complications. The duration of postoperative recovery after such interventions is about 10 days. Already on the fourth day the patient was able to move around and is now preparing for discharge. The patient expresses deep gratitude to the doctors who saved his life.
According to the head of the department, today specialists perform almost the full range of cardiac surgical interventions for the population of the republic and continue their development.
We are constantly working to increase the number and improve the quality of our operations. So that patients do not have to go to the mainland to some federal institutions and receive full-scale cardiosurgical and cardiovascular care here in the republic. Thus, last year we performed over 500 open-heart surgeries with cardiopulmonary bypass. This year we are planning to introduce new surgeries for Crimea, for example, plastic correction of the mitral valve. This is a complex operation that is used for coronary heart disease, post-traumatic detachment of the chorda of the heart, infective endocarditis— Ivan Davydov summed up.
