You need to know: which doctors are entitled to additional payments?

Incentive payments due to medical workers for identifying cancer in patients during medical examinations or preventive examinations of the population are intended to be extended for one year — until 2026 inclusive. The corresponding bill, prepared by the Ministry of Health, was published on the federal portal of draft regulatory legal acts. Promoting cancer vigilance

The bill amends the Law “On Compulsory Health Insurance in the Russian Federation,” which currently provides for the distribution of transfers for expenses for these purposes for 2020-2025. If the document is approved, it is assumed that the current standards will apply to 2020-2026, and the law itself will come into force on January 1, 2024. As noted in the explanatory note, the measures are aimed at eliminating the personnel shortage in medical organizations in relation to doctors and nursing staff.

For each cancer disease detected for the first time during medical examination and medical examination (as well as subsequently confirmed) doctors are entitled to payments; they were established back in 2020. They are calculated as follows:

  • 500 rubles — to the doctor responsible for conducting medical examinations and medical examinations;
  • 250 rubles — to the health worker who referred the patient for examination by an oncologist;
  • 250 rubles — to a medical worker who has carried out timely establishment of dispensary monitoring of a patient with oncology.

These amounts are paid monthly, the funds for which are distributed among medical organizations by territorial compulsory health insurance funds (MHIF).

It is very important that oncology is detected as quickly as possible if a person has it,” said Inna Svyatenko, Chairman of the Federation Council Committee on Social Policy, in a conversation with Parliamentary Newspaper. — That is why the motivation system for the initial detection of oncology during a preventive examination or clinical examination is truly extremely relevant.” According to her, this directly affects the life expectancy of patients and their recovery.

In addition, in the bill, the Ministry of Health also proposes to extend for a year, until 2026 inclusive, the provision of inter-budgetary transfers from the federal Compulsory Medical Insurance Fund budget to the territorial Compulsory Medical Insurance Fund. Transfers are allocated to co-finance the costs of medical organizations for the salaries of doctors and paramedical personnel, as well as for the costs of the above payments to health workers.

Other support measures

Healthcare workers can expect to receive other incentives as well. For example, from the beginning of 2023 a special social payment is intended for them. Its size varies from 4.5 to 18.5 thousand rubles, depending on the category of health worker and type of medical organization. For example, the following are established as maximum monthly amounts:

  • 18.5 thousand rubles — to doctors of central district, district and district hospitals, 8 thousand rubles — to specialists with secondary medical education, and 4.5 thousand rubles — to junior medical staff;
  • 11.5 thousand rubles — to doctors of emergency medical care stations (departments), 7 thousand rubles — to paramedics and nurses, 4.5 thousand rubles — to junior medical staff.

The payment is calculated taking into account the employee’s performance of his labor functions under the contract, as well as based on the total hours worked for the days of work in the corresponding month. To receive it, health workers do not need to do anything; it is provided automatically based on data from medical organizations.

In August 2023, the Government expanded the list of categories of health workers who may qualify for additional payments. Now, salary increases are also given to specialists with higher non-medical education. In particular, employees of central district, regional and district hospitals are entitled to 18.5 thousand rubles monthly, and their colleagues in clinics, day hospitals and first-aid posts — 14.5 thousand rubles.

In addition, from July 1, 2022, a new procedure for social support for employees of medical organizations where patients with coronavirus infection are admitted. They are given the right to an additional payment to their salary in the amount of 25%; for its calculation, the following are taken into account: salary for hours worked, additional payment for part-time work, regional coefficients, as well as payments calculated on the basis of average earnings.

There is also a separate area of ​​support for doctors working in rural areas. Thus, within the framework of the “Zemsky Doctor” and “Zemsky Paramedic” programs, for work in villages, urban-type settlements, as well as in cities with a population of up to 50 thousand people, health workers are entitled to a one-time “lift”, and it is not necessary to move to the countryside. Compensation in the amount of 1 million rubles is intended for doctors, and 500 thousand rubles for paramedics, midwives, and nurses at paramedic stations. Moreover, if settlements are located in remote and hard-to-reach areas, then payments increase — up to 1.5 million rubles and 750 thousand rubles, respectively. For the Far East, Far North, as well as regions of the Arctic zone, their size is 2 million rubles and 1 million rubles, respectively.
